Underwood, 27, won video of the year honors for "Cowboy Casanova." She also grabbed the CMT performance of the year video for a live version of "Temporary Home."
"Awesome fans," Underwood said as she accepted the video of the year trophy. "Thanks for seeing me out on the tour. God bless you guys. You are the best."
Lady Antebellum also had a good night, winning group video of the year for "Need You Now."
Other notable winners included Brooks & Dunn, Keith Urban, Miranda Lambert, Luke Bryan, Blake Shelton and Trace Adkins.
"We were very much not expecting this," joked Kix Brooks as he accepted the duo video award for Brooks & Dunn's "Indian Summer." "We were not expecting this so much that Mr. Dunn has gone to the restroom – in Santa Fe, N.M. ... If you think I'm bringing you this award, Ronnie, you're crazy, man. This is just the kind of thing that will make 'em think we're breaking up the duo."
The ceremony, held at the Bridgestone Arena, was hosted by Kid Rock.

On next week's episode, the 26-year-old country star will portray a mysterious pharmaceutical sales rep named Tiffany who catches the eye of Ted (Josh Radnor), who at one point resorts to a cuddly pig in an attempt to win her heart.
"This is the cutest piggy ever!" she responds to his attempt to woo her.
"Were you always this pretty?" Ted asks. "Or are you going to give me that, 'Oh in high school I was shy and awkward line?"

The country music star will be proceeded by Queen Latifah, who will perform "America the Beautiful" during the pregame festivities at Sun Life Stadium on Feb. 7.
HOLLYWOOD, Calif. — Carrie Underwood says her southern influence will be in full force whenever her wedding day rolls around.
"I like big hair and eyeliner. I want my wedding day to be me, so I'll probably be rocking some big hair and some eyeliner," the newly engaged country star told People at the People's Choice Awards held Wednesday in Los Angeles.
Underwood, 26, got engaged last month to hockey star Mike Fisher. With both stars frequently on the road, the "Cowboy Casanova" singer said the couple stays in touch thanks to some handy technology.
"We're big texters," said Underwood, who will make her acting debut in a March episode of CBS' How I Met Your Mother. "We talk on the phone. I-Chatting, I don't know what we'd do without it to be honest."
While the pair has yet to pick a wedding date, Underwood said she's keeping her big day simple.
"[I want] a groom, good food and an open bar," she said.
